There are six main modes in The Void Rains Upon Her Heart, five of which have to be unlocked by completing certain dreams.
Story
Story Mode is the base roguelike experience of TVRUHH. After choosing your character, you are pitted against random monsters. After defeating the monster, she will give you gifts, which can be seen at the selection screen. The more Karma you have, the more options you have in the selection screen, and there will be more powerful Gifts. Monsters are hidden until you meet them for the first time and gifts are hidden until radiated. In the end you can choose a final boss you want to fight among the unlocked final bosses.
Quickplay
Quickplay is a mode where you can freely battle any radiated monster of your choosing at any level(provided you upgrade Quickplay). You have a limited amount of gifts you can equip before your score gets reduced and you can equip only radiated gifts. The higher the level, the more gifts you can equip without a score reduction. After you beat a monster in quickplay, you get medals based on how well you do, with a Radiant Medal being the ultimate prize.
Altered Story
Altered Story is a longer story mode at its core. After defeating what would have been the final boss, you instead meet Abyss, who can add more monsters to your run in return for sacrificing things to her. You can sacrifice half of your Health Bar, all of your Panic Attacks, or gifts. After this, you have 3-6 more random encounters before a second final boss encounter. Afterwards, your score is tallied, and medals are awarded, much like Quickplay. Final bosses options are picked randomly contrary to the story mod.
The Towers
The Towers are a mode in which you fight several monsters on your way up the tower. There are 6 "family" towers, each dedicated to a single monster family and 3 "random" towers vary in number of fights. You only fight monsters that you have already met, and meeting more will allow you to climb higher in family towers. This mode introduced blessings and burdens - temporary gifts that last a few battles and can help or hinder your ascending. Other types of gifts are scarce in the towers mode. On top of the random towers you fight a random final boss and on top of the family towers you fight a corresponding newborn monster.
Ultimate Tower
Endless Nightmare
Endless Nightmare is a mode where you fight monsters as their level is constantly increasing starting from level 1. Monsters don't drop power gifts or quick gifts.
Endless Stress
Between fights, you can spend your karma to purchase gifts. Monsters can drop karma gifts.
Endless Terror
Monster fear does not increase with level or stage.
